Hi Julie, http://www.health.gov.on.ca/english/public/forms/form_menus/ohip_fm.html Form 1442-84 This will be the biggest hurdle. The consensus amongst those of us who have had our applications approved is that your doctor needs to check off both boxes on the application at the beginning of Part 4A. After you send in the application you will have lots of time to work on the rest of the packet. It took ten weeks for my application to be approved, and OHIP contacted my Dr for more information. This is the reason you want to have a supportive physician on your side. In the meantime CLOS will contact you about a date for your surgery. When OHIP approves your request (we have to think POSITIVE!) they will contact your doctor and also contact CLOS to confirm that you will be covered for the surgery. between your decision and your Big Day. Good morning everyone, I am so excited that I finally got my patient information sent in. Love computers but when we really have to rely on them and they are not working it can really make ones life a little more difficult. I have a quick question- what do I do now? Do I wait to hear from CLOS that I have been approved before I carry on with my packet, how do they let me know? Actually I lied I have another question. I will be applying for my surgery to be covered thru OHIP. At this time I only have one comobidity, do they require that you have several?
